Golden State Psychological Services, Inc., doing business as Dr. Anna Levi & Associates ("we," "us," or "our"), operates this website and provides psychological assessment and therapy services from our office in North Hollywood, California. This Privacy Policy describes how we collect, use, share, and protect information through this website and our online intake form.
Scope
This policy covers information collected through this website, our online intake form, and email or telephone communications related to inquiries received through the site. It applies to anyone who visits the site or submits a form, whether or not you become a client of the practice.
This website privacy policy is separate from the privacy protections that apply to your clinical records once you become a patient. See "HIPAA and Clinical Records" below.
Information We Collect
When you fill out the intake form on our Contact page, you provide your name and phone number (required), the service you are interested in (required), and optional comments. If you email us at drlevi@gpsych.org or call our office, the contents of that message and any information you choose to share are also collected.
Please do not share clinical details in the intake form or by email. The intake form is for first contact only. Sensitive health, diagnostic, or psychological information should be shared during a scheduled call or appointment, not transmitted through this website or unencrypted email.
When you visit this website, basic technical information may be collected by us or our service providers, including your IP address, general approximate location, browser type, device type, operating system, the pages you visited, and the time and date of your visit. This site does not currently use cookies, tracking pixels, advertising trackers, or third-party analytics platforms. If we add any of these in the future, this policy will be updated and the change noted in the "Last updated" date.
How We Use Information
We use the information you submit to respond to your inquiry and schedule a call or appointment, determine whether the services you are asking about are a good fit and refer you elsewhere if they are not, maintain a record of inquiries for our own internal practice management, and comply with applicable law and respond to lawful requests from authorities.
We do not use information submitted through this website for marketing or advertising, and we do not sell or rent personal information to anyone.
When We Share Information
We share information only with service providers that operate the site (described below) and where contractually limited to processing on our behalf; when required to comply with law, respond to subpoenas or court orders, or protect our rights, safety, or property, or that of others; and with your consent, for any other disclosure not covered above.
Third-Party Services
We rely on a small number of third-party services to run this website. Their handling of information is governed by their own privacy policies, which we encourage you to review.
- Formspree processes submissions from our intake form and delivers them to us by email. Formspree may retain your submission, along with technical metadata about the request (such as IP address and browser), in accordance with its own privacy policy.
- Google Fonts is used to load fonts on this site from Google's content delivery network. Google may receive your IP address and basic request information when fonts are served to your browser.
- Google Maps may receive your request and IP address if you click the office address to open it in a map.
- Our email provider processes message contents for delivery and storage of replies sent from our office.
Children's Information
Many of our clients are children, and the intake form is generally completed by a parent or legal guardian on a child's behalf. We do not knowingly allow children under 16 to submit their own information through this website without parental involvement.
When a parent or guardian provides information about a minor through the intake form, we treat that information with the same care described in the rest of this policy. We use it solely to respond to the inquiry and discuss possible services. Once a clinical relationship is established, information about the minor is governed by HIPAA, California medical privacy law, and our practice's clinical-records policies, not by this website Privacy Policy.
If you believe a child under 16 has submitted personal information to us through this website without parental involvement, please contact us so we can promptly delete it.
Retention and Security
We keep intake-form submissions and related email correspondence for as long as reasonably necessary to respond to your inquiry, schedule services, and meet our recordkeeping obligations. Where a clinical relationship is established, clinical records are retained for the periods required by California law and our professional licensing board. Where no clinical relationship is established, we periodically delete or de-identify older inquiry records.
We take reasonable administrative, technical, and physical safeguards to protect information submitted through this website. No method of transmission over the internet is perfectly secure, however, and we cannot guarantee absolute security. As noted above, please do not transmit sensitive health information through the intake form or unencrypted email.
Your California Rights
Under the California Consumer Privacy Act, as amended by the California Privacy Rights Act ("CCPA/CPRA"), California residents have specific rights regarding their personal information. These include the right to know what categories of personal information we have collected about you, the sources of that information, the business purposes for collecting it, and the categories of third parties with whom we share it; the right to access the specific pieces of personal information we have collected about you; the right to correct inaccurate personal information; the right to delete personal information we have collected about you, subject to certain exceptions; the right to limit the use and disclosure of sensitive personal information, where applicable; the right to opt out of the sale or sharing of your personal information for cross-context behavioral advertising; and the right to be free from retaliation for exercising any of these rights.
To exercise these rights, please contact us using the information at the end of this policy. We will respond within the timeframes required by California law. We may need to verify your identity before fulfilling a request, particularly for access, correction, or deletion requests. Authorized agents may submit requests on a California resident's behalf with appropriate written authorization.
Sale and Sharing of Personal Information
We do not sell personal information collected through this website for money. We have not sold or shared personal information in the twelve months preceding the effective date of this policy.
HIPAA and Clinical Records
Once you become a client of the practice, information about your evaluation, treatment, and care is "protected health information" under the federal Health Insurance Portability and Accountability Act ("HIPAA") and California medical privacy laws, including the Confidentiality of Medical Information Act. The handling of that information is governed by our practice's Notice of Privacy Practices, which is provided separately at intake and on request.
This website Privacy Policy does not replace or modify our HIPAA Notice of Privacy Practices. Information you submit through the intake form, before any clinical relationship is established, is governed by this website Privacy Policy.
Changes to This Policy
We may update this Privacy Policy from time to time to reflect changes in our practices, services, technology, or legal obligations. When we do, we will update the "Last updated" date at the top of this page. Substantive changes will be highlighted on this page for a reasonable period after the change takes effect.
